The Simsbury Police Department is providing parents with an 'Emergency Child Information Packet'.  This packet is in an Adobe Acrobat^® format*.  Please print and complete this form with as much detail as possible.  Once completed, place the packet in a safe and easily accessible location in the unfortunate event of an emergency where a child is abducted or missing.

Be sure to print out an individual packet for each child.
